CLIA (Clinical Laboratory Improvement Amendments) is the federal regulation requiring all US laboratories testing human specimens to meet quality standards. CAP (College of American Pathologists) accreditation is a voluntary, more rigorous standard that includes regular inspections and proficiency testing. Of approximately 320,000 CLIA-certified labs in the US, only about 8,000 hold CAP accreditation. What Are the Different CLIA Certification Levels?
CLIA certification has three tiers based on test complexity, each with different quality requirements.
CLIA-waived is the lowest tier — covering simple tests like rapid strep, pregnancy tests, and basic glucose monitoring. Waived labs have minimal inspection requirements and do not undergo proficiency testing.
CLIA moderate complexity covers tests requiring trained personnel and documented procedures. Labs at this level undergo biennial inspections.
CLIA high complexity is the highest tier — covering advanced tests including comprehensive metabolic panels, hormone assays, and advanced lipid testing. High-complexity labs must maintain documented quality assurance, personnel qualifications, proficiency testing, and equipment calibration records.

Limitations and Considerations
- CLIA certification does not guarantee individual test accuracy. Accreditation covers the laboratory's quality systems, not the accuracy of every individual result. Anomalous results can occur in any lab.
- Accreditation covers the lab, not the collection method. Sample quality depends on both laboratory processing AND the collection technique used. EasyDraw arm collection, finger-prick, and venipuncture produce different sample quality levels regardless of which lab processes them.
